It is IMPORTANT that you fill out all information on this survey and be sure that all information supplied is accurate and reliable as this information will be used to contact you once vaccinations are in stock at the pharmacy. Once again, please provide a working and up to... date phone number and email address. This survey is open to the public, not just SEMO Drug patients. As of January 14, 2021, we do not have the vaccinations in stock, but are using this survey to plan accordingly and hope to get them in the near future. Orders have been placed. The state cannot promise providers an expected date of arrival at this time, but follow our Facebook page SEMO Drug of Kennett where we will provide updates as we get more information. Currently, as of January 14, 2021, Missouri is allowing the vaccination of individuals in Phase 1A (health care workers and LTCF residents and employees).This has been expanded also to individuals in Phase 1B -Tier 1 and shall allow those who are EMS workers, law enforcement employees, public service employees, and more to be vaccinated as well. On Monday January 18, 2021, the state is also opening up Phase 1B Tier 2 to all individuals 65 and older as well as specific health conditions. If you do not fall in these categories, you may still sign up, but will be called accordingly due to Missouri's COVID vaccination guidelines. Filling out this survey shall only take a few minutes. Please ONLY register ONCE. Registering multiple times WILL NOT increase your chance of getting the vaccination sooner!
