Unfortunately, just had a patient diagnosed with cancer after excision surgery. This is only my third patient in 12 years, over 3500 cases, with such a diagnosis. Hormonal suppression, as at best symptomatic treatment, not only can delay the diagnosis of endometriosis (as a tissue diagnosis), but also in the rare case of cancer (which matters if the patient is you). ... We published a similar case several years ago of an even younger patient who had cancer: https://journals.sagepub.com/doi/10.5301/je.5000201 Both these patients would have had later delay in diagnosis AND treatment of their cancer. I am privileged to be able to teach residents about surgery for endometriosis. Here we are using chopsticks to demonstrate the importance of triangulation. Also had the opportunity to tell a Chinese parable about heaven and hell... In the afterlife everyone uses long chopsticks. The girls can’t wait to hold him and brother Max can’t wait to play with him. I tell patients ‘we are all in this together’, because it is true. ... My wife had debilitating periods her whole life and we dealt with infertility for 4 years. After optimal excision surgery for Stage IV endometriosis (with giant bilateral endometriomas, and we were told that we would never get pregnant but through IVF) and several surgeries for adhesion prevention, she now feels great, and has had recurring fertility. Through her experience I have learned much about the value of optimal excision, the importance of adhesion prevention, and the difference a caring provider/surgeon can make. We owe a debt of gratitude to many who have helped us along the way in different ways, but in particular, to Dr. Ken Sinervo @kensinervo (and @wwiner) at the @centerforendocare, who mentored me, and who did her first and last surgeries. He is truly a mentor, example, and friend. We have to do better for women with endometriosis!
