Another one of our staff members has started training to become a Chronic Disease Self Management Program leader. CDSMP is an evidence-based program that was originally developed at Stanford University. Because our health care providers can't be with us everyday, we have to learn how to self-manage our chronic diseases. CCSS is providing leadership to the Northland Living Well Collaborative to offer and coordinate this program across Clay County.

November is Caregiver Appreciation Month Tri-County Mental Health and Northland Shepherd's Center offer Caregiver Support Groups virtually from 10:30 am til noon on Thursdays, November 10 & 24; on Tuesdays, December 8 & 22. After some lighthearted sharing of our favorite childhood Halloween costume, we heard from Linda Ward of Center for Practical Bioethics who explained some really important reasons to have discussions with family members about advance care planning.
