Are you dealing with Sciatica? What you need to know and how to solve it . 1 Roughly 13 40% of people will deal with an episode of sciatica in their lifetime, and most incidences of sciatica resolve within a couple of weeks. However, if left untreated sciatica can last for months to years. . 2 We often think sciatica as nerve pain referred down the leg from the low back but this is not always the case sciatica is actually a catch all term for any pain arising from ...the length sciatic nerve. . 3 Many common sites of sciatica include the low back, the sacroiliac region, the buttocks region, and even behind the knee therefore it is important to get assessed before assuming the actual cause or source of pain. . 4 Should you be stretching? stretching is often thought of as one type of intervention that can help with sciatica and while it can be beneficial it is often only one small piece of the puzzle when helping to reduce pain and discomfort. Sometimes stretching would be contraindicated and could increase a patient’s symptoms. . 5 How to solve it Get assessed so you can rule in or out possible areas of your body that could be contributing to your symptoms ( low back, sacroiliac joint, the hip and the leg ) and then get a program specifically tailored to you. Any intervention stretching, strengthening, or modifying daily tasks is only as good as the assessment. . Two more nuances of squatting you will want to know for a better squat: Starting alignment matters and hip mobility is a combination of the pelvis acting on the leg and the leg acting on the pelvis. You will have more relative hip flexion the less you go into an anterior pelvic tilt and vice versa (more posterior pelvic tilt the less relative hip extension). If you are having difficulty squatting deep it might because you are losing your pelvic position early in your squat.... . While knee collapse is not ideal, it should not be demonized either do not train into knee valgus but understand many elite lifters use knee valgus to help complete a lift. Also, the injury mechanism for knees, often includes forced knee hyperextension on top of a collapsed position (you don’t see this in squatting). . Deep hip external rotators, hip stability, and hip pain . Much like the rotator cuff muscles at the shoulder, one primary function of the deep hip external rotators is to assist in joint approximation. . Also, because the hip is a ball and socket joint, there is decreased bony congruency to allow for functional ranges of motion. I.e., less joint congruency allows for increased range of motion or think in terms of the stability versus mobility spectrum.... . Due to this, other areas of the hip joint must pick up the slack (see our prior post), but think in terms of the labrum, joint capsule, and passive and active tension in muscles. . As a follow up to our previous post, if someone has too much motion at the hip joint or decreased passive stability from an injury (labral tear), one way we can help provide joint congruency is through dynamic activation of the deep hip external rotators. . These muscles sit very close to the hip joint and help assist in joint approximation. The 3 nuances of squatting you’ll want to fix for a better squat: Choosing a squatting style that does not fit your body structure You cannot and fit a square peg into a round hole, and the same goes for squatting. Bony variations of the hip, knee and ankle must be accounted for before ever choosing a squatting style or worrying about which muscles are working. Everyone’s squat should look slightly or dramatically different because of this. The amount of effort needed for... your squat should match the intensity of the exercise (bodyweight vs 1 RM) there is an inherent interplay between mobility and stability, the more you tense up and create stability the more you may have to fight to get range of motion in your squat. The foot should pronate. In reference to above, while we want a stable foot, stability is often over cued at the expense of mobility as your knee comes over your toe, as with walking or jumping, foot pronation helps assist the normal mechanics of ankle dorsiflexion. If your dorsiflexion is limited it might be because you are too ridged in your foot position. We are not all built the same, therefore we should not all strive to squat the same. Every single person’s squat should be a variation of the squat movement.... . 3. As life happens, we develop adaptive changes (loss of joint range of motion, hypermobility, learned behaviors, etc.). Knowing your body's strengths, weaknesses and limitations is a huge factor for determining your ideal squat and keeping you healthy. . 4. Anthropometric measurements (body proportions) matter and if your struggling with a lift, it might because your performing the lift like your 5’9 when you should be performing it like your 6’1. This matters even more so when dealing with injury. .
