Attention: Location Change for COVID-19 Vaccine Second Dose Clinic This Saturday, February 13, 2021 Due to adverse weather conditions, the Phelps Health COVID-1...9 Vaccination Second Dose Clinic scheduled for this Saturday, February 13, 2021, from 8:00 AM to 5:00 PM, has changed location from the Phelps Health/Missouri University of Science and Technology parking lot to the S&T Gale Bullman Multipurpose Building, 705 West 10th Street, Rolla, MO. This second dose clinic is ONLY for individuals who already received their first dose of the Pfizer vaccine on January 23, 2021. Individuals scheduled to receive their second dose on Saturday, February 13, should park and enter on the west side of the S&T Gale Bullman Multipurpose Building. Please remember to bring the following items with you: -- COVID-19 vaccination card -- Photo ID -- Insurance card To ensure we are able to maintain social distancing, please do not bring additional people into the facility unless you require assistance.
