There are many things in life I have taken for granted, but the opportunity and ability to be a mother will never be one of them. As National Infertility Awareness Week 2021(#NIAW2021) kicks off, I thought I would share our beautiful rainbow baby Weston. A pandemic pregnancy and rainbow baby, most people say ‘I didn’t even know you were pregnant! Yes. My coping mechanism was double masking in clinic for 10 months and silently hoping and praying for a healthy baby and d...elivery free of Covid and more loss. My classic retort when my STLFertility team asked if I was going to tell my patients I was pregnant was I will tell everyone I was pregnant when the baby is here safe and sound. It wasn’t because I didn’t want to shout it from the rooftops and enjoy every second, it was out of compassion for my struggling patients, and fear of the unknown. Will this pregnancy stick? Two prior hadn’t. If I get Covid - what will happen to the baby? I was a physician looking for evidence based answers to unanswerable questions in uncertain times. I am happy to report that baby Weston is here safe and sound. We are forever blessed and my message today for everyone struggling with infertility, loss or uncertainty is YOU ARE NOT ALONE. Find someone you can share your fears with and who will listen and not judge. Find coping mechanisms that aren’t harmful (exercise / double masking =) / keeping your pregnancy to yourself until you’re comfortable, even with a basketball belly!!!) and most importantly - KEEP THE HOPE ALIVE. WHO acknowledges that COVID vaccine trials excluded pregnant women and therefore they advise pregnant women to speak with their healthcare provider before becoming vaccinated. The BENEFIT of the vaccine may outweigh the unknown theoretical RISKS in pregnant women. Preliminary studies in pregnant animals have shown no harmful effects. . . Essentially that is what the article on the left says if you read it in it’s ENTIRETY and then the NEW headline/article on the right FROM TODAY ALSO says . . . . . . I'm pregnant or breastfeeding - should I get Pfizer’s COVID vaccine? . .... Short answer? It depends. Consider getting the vaccine if you are pregnant and or breastfeeding and in a ACIP recommended priority group (ie healthcare workers). . . . The vaccine has not been tested in pregnant women, therefore there are no safety data specific to use in pregnancy. . . However - the American College of Obstetricians and Gynecologists (#ACOG) recommends that pregnant patients who are in the PRIORITY groups (ie - healthcare workers and residents of long-term care facilities) outlined by the Advisory Committee on Immunization Practices (ACIP) and approved by the CDC, be eligible to receive the vaccine. . . They recommend a full discussion with your physician/clinical care team prior to obtaining the vaccine but do not require it. . . There is no safety data on the use of COVID vaccines in pregnancy but there is also no data to indicate that the vaccine should be contraindicated. THEREFORE ACOG recommends pregnant women should be free to make their own decision in conjunction with their clinical care team. . . When deciding, consider the following: . 1. The activity of the pandemic in the community 2. The potential efficacy of the vaccine 3. The potential risk and severity of maternal disease including the effects of disease on the fetus and newborn 4. Safety of the vaccine for the pregnant patient and the fetus . . Remember - mRNA vaccines are NOT LIVE VIRUS VACCINES. They do not enter the nucleus and do not alter human DNA in vaccine recipients. They can’t cause any genetic changes. . . . Additionally, to address the main side effects of the vaccine and size of the studies surrounding it - the body of evidence for the Pfizer Vaccine was primarily obtained from one large randomized double blind, placebo-controlled clinical trial that enrolled >43,000 people. Among recipients, 8.8% reported most common side effects were fatigue, headache, muscle pain, chills and injection site pain. . . Hope this helps. . . . Joining my colleagues in fighting misinformation around the Pfizer #Covidvaccine causing #infertility. .... . Experts agree that there is NO EVIDENCE TO BACK UP THIS CLAIM. . . These rumors about infertility were started after an article was published which falsely claimed that the vaccine’s ability to induce the spike protein of SARS-CoV-2 is also capable of eliciting an immune response training the female body to attack a different protein (syncytin1) that is associated with placenta formation. This is because the two proteins (spike protein and syncytin1) look similar. Meaning that they share a small amino acid sequence. This led to the concern for infertility/miscarriage. . . . Immunologists and experts in Maternal and Neonatal Immunity have weighed in and pointed out that coronavirus spike protein and the placental protein in question have ALMOST NOTHING IN COMMON. Additionally, the amino acid sequence is too short to plausibly give rise to autoimmunity. . . . The New York Times article stated mixing them up would be akin to mistaking a rhinoceros for a jaguar because they are wearing the same collar. (which I thought painted a very clear picture of how different they are) . . . INITIALLY there were some reports of increased pregnancy loss in COVID patients however as the pandemic rages on, many larger studies have shown NO ASSOCIATION. So we are NOT seeing more miscarriages in COVID infected patients which also reassures us that this claim about the vaccine is not true. . . . This situation is fluid and researchers are continuing to monitor the well-being of vaccinated people. . . My version of a Magic Wand this Halloween! . .... This is a transvaginal ultrasound probe. Transvaginal ultrasounds are so important to Fertility Doctors because these ultrasounds provide information about pelvic anatomy. They truly are the magic behind helping us build families for our patients. . . When we perform an ultrasound, we evaluate the uterus and ovaries and the adnexa and determine if there are any adnexal masses adjacent to the ovaries or uterus. We count the number of small follicles in each ovary which is called an antral follicle count. This gives us an idea of a woman’s ovarian reserve. . . . Got to see this MAMA to be yesterday! Everyone’s infertility journey is different and this brave warrior overcame intrauterine adhesions also known as Asherman’s syndrome. .... . The uterus is a very dynamic organ and the part of the uterus that sheds during your period is called the endometrium, the outer muscle layer is called the myometrium. In some cases, surgery on your uterus (a D&C for example) can lead to scarring and formation of thick tissue bands that connect the anterior and posterior (top and bottom) parts of the uterus and affect the endometrium. Sometimes spotting for menses or even amenorrhea (no periods) results which is a tell-tale sign that you could be suffering from Asherman’s Syndrome. This scarring can prevent pregnancy and render women sub fertile. . . Treatment? Surgery (sometimes multiple surgeries are required) and occasionally the addition of hormonal therapy. The goal is to restore the inside of the uterus to its natural shape without the thick tissue bands and restore the endometrium. . . This brave warrior underwent all of that on her journey and proudly stands wearing her MAMA shirt, expecting and excited. . . COVID precautions are still in full effect to protect our patients and our staff. . .... One of our precautions to limit exposure is allowing only patients into our office. This means that patients come in solo for their embryo transfers. One of the many letdowns during this pandemic, partners not being in the room for the culmination of their fertility journey can be devastating. . . While we use FaceTime and video to help the partner feel like they are there, my creative patient took it one step further......she created this AMAZING shirt displaying photos of her hubby’s face........(not Ryan Gosling’s although #doppelganger) . . Full of energy and passion, she has been such a pleasure to have in the office. She handles herself with grace and poise, asks appropriate questions and has been AMAZINGLY helpful. . . No doubt in my mind that she will EXCEL in the ob/gyn residency of her choosing and my hope is that she goes on to fellowship in REI. It is such an honor to give back to the next generation of medical students. I remember all those wonderful physicians who supported me on my path and I’m proud to now be on the other side. . . .
